Due care, conflicts and escalation

- Choose a documented response to an independence conflict or urgent unsafe observation without claiming authority beyond the audit mandate.
Professional care is an active discipline, not a disclaimer attached to a report. Before accepting an assignment, list financial, design, reporting-line and prior-decision connections with the activity. A person who approved the procedure being audited cannot cure that conflict by declaring personal objectivity. The audit sponsor should assess reassignment, independent review or a scope change, and the decision should be recorded. During work, distinguish a confirmed observation from an allegation. Trace the applicable criterion and evidence, protect confidential identifiers, and avoid distributing an unverified accusation. The ASQ ethics source guides honesty, competence, confidentiality and conflicts; it does not grant an auditor regulatory enforcement powers.
A suspected immediate danger needs prompt communication through the site's safety process, even if the formal audit finding will require later corroboration. Capture time, place, what was seen and whom it was reported to. Do not operate unfamiliar machinery, order a shutdown outside delegated authority, or wait for a polished report before alerting the responsible person. If the situation may involve illegality, do not turn suspicion into a legal conclusion; preserve the record and use the organization's escalation channel, seeking qualified advice. Careless statements can harm people and organizations. A conclusion should say whether the record establishes nonconformity, supports further investigation, or leaves a material uncertainty. The audit trail must also reveal how a conflict was managed so that an independent reader can judge credibility.
Use a two-track note in a sensitive event. The safety track records the exact observation, location, operating state as known, notification time, recipient and protective action taken by the responsible site role. The audit track records the effective criterion, sample, later corroboration and finding decision. The auditor must not write that a safety action was performed merely because a call was made. In parallel, a conflict register states the lead's prior design decision, affected scope, sponsor's disposition and who independently reviewed conclusions. If a confidential photograph is needed, record permission, access and retention rather than attaching it to a broad meeting invitation. A disputed fact may be corrected without erasing the first note: retain the original, amendment and reason. This structure protects urgent communication and procedural fairness. It also lets the team distinguish poor judgment from the separate legal question of negligence, which needs qualified analysis beyond the facts of a training case.
