Public Health Foundations, Determinants and Prevention · Lesson 2

Determinants of Health, Equity and the Distribution of Health Outcomes

14 min read

Lesson objectives

  • Explain how social, environmental and organizational conditions shape health opportunities without reducing them to individual traits.
  • Analyze a plausible pathway from determinant to outcome and identify what requires evidence.
  • Use an equity lens to examine differences in access and opportunity to benefit across groups.

Why is health not distributed equally?

Health is not shaped only inside a clinic. Housing, education, income, work, transport, food, environment, safety and access to services can influence people’s opportunities for health. Public health often groups many of these conditions under “determinants of health.” The important skill is not memorizing a fixed list but tracing pathways: how might a social or environmental condition change exposure to risk, access to a protective resource, or the ability to benefit from a health service? WHO includes the conditions in which people are born, grow, work, live and age, together with wider forces that shape those conditions.

When examining health distribution, distinguish a difference from an equity concern. Health outcomes can differ for many reasons, and not every difference is easily modifiable. An equity lens asks whether disadvantages are linked to unfair, avoidable or remediable conditions. In this diploma, equity is used as an analytic question: who faces a larger barrier? Can everyone benefit from the intervention to the same degree? Could a one-size-fits-all design leave one group behind? This does not justify assuming a cause or motive; it requires examining data and context.

Type of factorAnalytic questionNon-clinical example
Social/economicDo resources, income or education change the ability to benefit?Transport cost limits access to a preventive service
EnvironmentalDoes the setting increase exposure or reduce protection?Heat, pollution or crowding in a setting
OrganizationalDoes service design create a barrier?Opening times that exclude some workers
Wider structuralDo policy or resource patterns shape opportunities?Services distributed unevenly across areas

Lesson summary

Determinants are conditions and pathways that shape exposure, resources and access. Equity analysis asks who faces larger barriers and who may be left behind, while avoiding causal overclaiming or turning group patterns into individual judgments.
